Warsaw Meadows Care Center is a privately-owned, comprehensive skilled nursing facility offering a full spectrum of care including rehabilitation, short-term Sub-Acute care, long-term care, and memory care. We provide 24 hour medical care and support services to our Residents.

Through collaboration with community resources and support to our families, we ensure that our Residents achieve their highest level of independence. Here at Warsaw Meadows Nursing and Rehab, you will find all the comforts of home with a caring and friendly staff. We encourage you to decorate just like you would at home within our spacious, clean environment. Family, friends, and pets are welcome to visit anytime.
